Report Writing

Each Shift is required to prepare a Daily Guard Report for each their shift. This report must be completed in detail.

What To Report: Report all safety and fire hazards, equipment in need of repair or equipment which might affect facility security, suspicious activities in and around your post, fires, and unusual incidents involving employees or visitors.

Contents: Your Daily Guard Report must contain the information necessary to make it understandable to the Client and your Supervisors. Please use the following steps when preparing:

  • Who - Include the names, addresses and telephone numbers of persons involved in any incident, or, a good description of the person.
  • When - Include the time and date of the incident, or when you became aware of it.
  • Where - Write the location of the incident or situation requiring attention.
  • What - Objectively give a detailed explanation of what occurred or what you observed. If you are stating an opinion and not fact, indicate that it is an opinion.
  • Why - Give a complete explanation as to why something happened. If you cannot explain, indicate that you have no idea(s).
  • How - Indicate your observations regarding the reason an incident occurred. Your opinion at this point is welcome and may give the client the answer as to what measures can be instituted to prevent a reoccurrence.
  • When - a door is found unlocked, log in Guard Report and enter information into data recorder.
  • Please make your reports legible, accurate and thorough. Print your reports if necessary.
